- Identify and monitor safety issues within the person’s served home.
- Support and closely coordinate behavioral health services with the person’s served psychiatric team.
- Monitor DSPs to evaluate correct administration of medications and treatments is occurring as prescribed and in accordance with medication manager regulations along with agency policy and procedures. Review/investigate any medication errors and follow agency policy and procedure for proper coaching and/or disciplinary action needed.
- Direct provision of first aid and arrangement of emergency medical appointments/care when necessary.
- Ensure fire and emergency drills are scheduled, completed and documented as required by agency policies and procedures.
- Coordinate and facilitate annual planning meetings with persons served.
- Facilitate changes to the ISP when needed for persons served who are making progress quickly or for those who are not recovering as expected.
- Assist with the ongoing development of services based on the needs of the persons served – including: intake and referral process, securing housing and assuring the appropriate level of support.
- Ensure services emphasize evidence-based and emerging best practices to produce outcomes congruent with personal recovery and needs of each of the persons served.
- Advocate for persons served in the community while maintaining open communication, both written and oral.
- Assist persons served with activities of daily living, physical health-care and maintenance tasks, etc. as needed.
- Teach and coach DSPs in the most effective approaches to achieve direct support competencies. Align them with appropriate trainings applicable to their intellectual disability, mental illness and/or brain injury: and best practices when working with persons served who experience symptoms of mental illness or who are survivors of a brain injury.
- Coordinate, schedule and document DSP participation and performance in orientation and in-service trainings and completion of other alternative self-directed learning and development.
- Observe and solicit feedback from DSPs, persons served and their families regarding training needs and desired opportunities.

- Must be 18 years of age or older.
- Ability to coordinate and provide oversight for multiple sites providing services to person served with Intellectual Disabilities, Serious Mental Illness (SMI) or Brain Injury (BI) through: demonstrated supervisory and leadership skills, budgetary management skills and ability to develop, organize and coordinate activities related to the persons served and organizational goals.
- Ability to manage multiple tasks.
- Ability to work effectively with people.
- Demonstrated ability to represent the organization and the persons served in a professional manner: exemplary written documentation, clear and professional verbal communication and ability to work effectively with internal and external stakeholders.
- Ability to communicate professionally utilizing clinical language.
- Knowledgeable of community resources, information and referral practices and problem-solving techniques to best assist persons served.
- Ability to work flexible hours including mornings, evenings and weekends.
- Ability to travel in order to participate in events, training sessions and meetings throughout the region and state.
- On-Call supervisor responsibility will be determined by the Program Director.
- Demonstrated ability to communicate effectively, both orally and in writing.
- Ability to speak, read and write in English.
- This position requires a valid driver’s license, reliable vehicle, good driving record, and current vehicle insurance coverage, which meet the specified requirements of EPI’s Driving Policy.
Qualifications:
- Bachelor’s Degree in Human Services or related field.
- Three (3) years of experience in developing, coordinating and delivery of disability services or equivalent. Equivalency may be established through a combination of work experience and training.
- This position requires a valid driver's license, and the ability to pass an extensive background check, including checks of the Dependent Adult and Child Abuse registries, the Sex Offender registry, and a criminal history check.
