We are currently seeking a highly qualified and compassionate Social Worker to join our well-respected team in our Waterloo office.
Job Overview
Title: Social Worker
Location: Waterloo, Iowa (may be assigned to help at any CVH location as needed)
Job Schedule:
- Full Time (1.0 FTE)
- Monday – Friday, 8:00am – 4:30pm
- Cedar Valley Hospice offers flexible work arrangements to work around the needs of your family! Four 10-hour days and adjusted work hours are available. (Flexible work arrangements available after 90 day probation period.)
- This position will also be part of an on-call rotation for either weeknights or weekends and also for holidays.

Responsibilities
- Provide and document assessments of the psychosocial statuses of patients/families.
- Develop and implement goal directed interventions based on assessment findings and patient/family needs.
- Provide Case Management to assist patients/families in accessing community resources.
- Participate in developing the individualized patient plan of care.
- Serve as liaison between social and community agencies and the hospice program.
- Attend interdisciplinary team meetings and participate in developing patients’ plans of care.
Qualifications
Required
- Bachelor’s degree in Social Work from an accredited program (BSW) with one year experience in a health care setting.
- Positive attitude and team player.
- Demonstrate good written and verbal communication skills.
- Basic computer skills with the ability to learn work-related software and have basic typing skills.
- Current Iowa driver’s license, reliable vehicle, and car insurance.
Preferred
- Master’s degree in Social Work from an accredited program (MSW).
- 2 years of recent experience as a Social Worker in a related field.
- LMSW/LBSW licensure
- Experience working in a team environment.
Physical/Other Requirements
- Ability to move 50lbs with or without accommodation.
- In an 8-hour day, employee may remain in a stationary position 1-5 hours and move about inside CVH site offices/locations, meeting spaces, patient residences, and healthcare facility locations 3-7 hours (or vice versa).
- Employment at Cedar Valley Hospice requires being fully vaccinated for COVID-19 (Boosters are not required) OR have an approved request for reasonable accommodations for a medical, pregnancy, or religious reason, by the start date.
- Pre-employment/post-offer physical, TB, and drug screen required.
