Beginning in July, we will take part in a new Medicare initiative we’ve been chosen to provide for our community: the GUIDE Model. With the accelerated rate of dementia in our population, this one-of-a-kind program is aimed at supporting families and caregivers navigating the challenges of dementia-related diseases.
